How can I refer someone to receive meals on wheels, homemaker aide, Respite services or Community Care?
All referrals are made through the Lower Chattahoochee Area on Aging. They may be reached at 706-256-2900 or 800-615-4379. A phone screening reflecting the needs of the elderly or disabled persons will be assessed. A referral form may also be downloaded and faxed to the Area Agency on Aging. The services are rendered by greatest need and referrals are then made to Direct Service Corporation.
Who would qualify to receive help from Direct Service Corporation?
The answer depends on the type service needed. For some programs, a client MUST be 60 years of age and over. For other programs, a client MUST have a physical or mental disability or disorder which restricts his/her ability to perform basic activities of daily living and/or instrumental activities of daily living, or which threatens his/her capacity to live independently. Other programs require both of these criteria to be met. The Lower Chattahoochee Area on Aging will make the determination as to which service best fits the clients need.
Is there any charge for services?
The Lower Chattahoochee Area on Aging will determine any cost-share per client income.
How long does it take for someone to begin receiving services?
Because of the great need in our community, there is a waiting list in most areas of elderly services. Depending upon the needs, this wait could be several days or several months. Once a referral is made from the Area Agency on Aging to Direct Service Corporation, a home visit is made within three days to begin the client on the progam.
I have already contacted the Area Agency on Aging, and feel the waiting list is too long. How can I receive services immediately?
